Consent to the processing of personal data
I hereby consent to the processing of my personal data in the form of: first and last name; street; house number; apartment number; postal code; city; country; contact telephone number; e-mail; company name; tax identification number, for the purpose of providing services to me by the Administrator Luxorna Tomasz Lis, NIP 6991465541, Czerminek 2H, 63-322 Gołuchów.
I declare that, pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), I have been informed that:
- the controller of my personal data is Luxorna Tomasz Lis, NIP 6991465541, Czerminek 2H, 63-322 Gołuchów,
- my personal data will be processed solely for the purposes indicated above and will be disclosed to third parties, whereby the recipients of the data will be entities providing services to the controller - on the basis of data processing agreements,
- my personal data is processed for the purpose of performing the agreement concluded between me and the controller and, on the basis of my consent, for the purpose of direct marketing,
- my personal data will be stored for the duration of the agreement concluded between me and the controller,
- I have the right to request from the controller access to my personal data, its rectification, erasure, or restriction of processing, I have the right to object to processing, and I have the right to data portability,
- I have the right to withdraw my consent to the processing of my personal data without affecting the lawfulness of processing carried out on the basis of my consent before its withdrawal,
- I have the right to lodge a complaint with a supervisory authority,
- the provision of personal data is voluntary, and failure to provide it results in the inability to perform the agreement concluded between me and the controller,
- the controller collects data in an automated manner but does not apply profiling to me.
- I declare that I am of legal age and have full legal capacity. I also declare that I have read the full content of the above consent and fully understand it.
